* Service Projects and The CREW Club participation requires an Activity Consent and Release Form (Click link for form)

* Lead Activity Coordinate for a service project or The CREW Club will need prior approval. Submit an Event Request Form at least 2 weeks before the activity. (Click link for form)

* Activities where volunteers and/or overnight attendees participate in youth events will require a Washington State Patrol background check. Submit a Volunteer and Resident WATCH Form (Click link for form)

* Activities where volunteers may be transporting participants requires a Driver Information Form (Click link for form)

* For pre-approved expenses that need reimbursement, complete the Reimbursement Out of Pocket Expense Form (Click link for form)
